ABSTRACT

Background and aims: Cerebral venous thrombosis (CVT) is a rare complication of COVID-19 vaccines, which has been extensively covered by lay media fueling fear and vaccination skepticism. We assessed the impact of this media release on neurological emergency care. Methods: We performed a chart review of consecutive people presenting to the neurological emergency room of our university hospital during 30 days before the first national press release on COVID-19 vaccinerelated CVT on March 14th, 2021 with those who presented during the subsequent 30 days. Results: Among 622 patients during the total observation period, we noted an increase in the proportion of those presenting with headache (47/276 patients (17.0%), 60% females, ages 41 [30-61] median [IQR] preannouncement versus 84/346 patients (24.3%), 65% females, ages 43 [35- 63] post-announcement, p=0.03). Cardiovascular comorbidities (p=ns), neurological deficits (p=ns), imaging results (p=ns) did not differ between time periods (p=ns). None of the patients in both time periods had a diagnosis of CVT or vaccine-induced thrombotic thrombocytopenia, but fear of CVT was more prevalent after the press release (n=0(0%) vs. 32(38.1%), p<0.0001). Patients with fear of CVT presented 11 [8.5-16] days post vaccination (87.5% ChAdOx1 nCoV-19, 12.5% BNT162b2), and none of those had pathological findings on cranial imaging. On multivariate and multinominal regression analyses those with fear did not differ in the odds of undergoing cranial imaging from those without fear (OR 0.7, 95%-CI [0,26-1.87], p=ns). Conclusions: Fear of CVT after COVID-19 vaccination seems to engender additional availment of neurological emergency care to a relevant extent.

ABSTRACT

Objective: The objectives of this study were to measure the global impact of the pandemic on the volumes for intravenous thrombolysis (IVT), IVT transfers, and stroke hospitalizations over 4 months at the height of the pandemic (March 1 to June 30, 2020) compared with two control 4-month periods. Background: The COVID-19 pandemic led to widespread repercussions on the delivery of health care worldwide. Design/Methods: We conducted a cross-sectional, observational, retrospective study across 6 continents, 70 countries, and 457 stroke centers. Diagnoses were identified by ICD-10 codes and/or classifications in stroke center databases. Results: There were 91,373 stroke admissions in the 4 months immediately before compared to 80,894 admissions during the pandemic months, representing an 11.5% (95%CI,-11.7 to-11.3, p<0.0001) decline. There were 13,334 IVT therapies in the 4 months preceding compared to 11,570 procedures during the pandemic, representing a 13.2% (95%CI,-13.8 to-12.7, p<0.0001) drop. Interfacility IVT transfers decreased from 1,337 to 1,178, or an 11.9% decrease (95%CI,-13.7 to-10.3, p=0.001). There were greater declines in primary compared to comprehensive stroke centers (CSC) for stroke hospitalizations (-17.3% vs-10.3%, p<0.0001) and IVT (-15.5% vs-12.6%, p=0.0001). Recovery of stroke hospitalization volume (9.5%, 95%CI 9.2-9.8, p<0.0001) was noted over the two later (May, June) versus the two earlier (March, April) months of the pandemic, with greater recovery in hospitals with lower COVID-19 hospitalization volume, high volume stroke center, and CSC. There was a 1.48% stroke rate across 119,967 COVID-19 hospitalizations. SARS-CoV-2 infection was noted in 3.3% (1,722/52,026) of all stroke admissions. Conclusions: The COVID-19 pandemic was associated with a global decline in the volume of stroke hospitalizations, IVT, and interfacility IVT transfers. Primary stroke centers and centers with higher COVID19 inpatient volumes experienced steeper declines. Recovery of stroke hospitalization was noted in the later pandemic months, with greater recovery in hospitals with lower COVID-19 hospitalizations, high volume stroke centers, and CSCs.

ABSTRACT

Introduction: Recent studies linked coronavirus disease 2019 (COVID-19) to thromboembolic complications likely mediated by increased blood coagulability and inflammatory endothelial impairment. Objective: We aimed to assess the risk of acute stroke in patients with COVID-19 related to clinical severity of the disease. Methods: We conducted an observational multicenter cohort study in four participating hospitals in Saxony, Germany to characterize consecutive patients with laboratory-confirmed COVID-19 who experienced acute stroke during hospitalization. Furthermore, we performed a systematic review using PubMed/MEDLINE, EMBASE, Cochrane Library and bibliographies of identified articles following PRISMA guidelines including data from observational studies of acute stroke in COVID-19 patients. Data was extracted by two independent reviewers and pooled with multicenter data to calculate risk ratios (RR) and 95% confidence intervals (95%CIs) for acute stroke related to COVID- 19 severity using random effects model. Between-study heterogeneity was assessed using Cochran's Q and I -statistics. PROSPERO identifier: CRD42020187194. Results: Of 165 patients hospitalized for COVID-19 (49.1% males, median age 67 [57-79], 72.1% severe or critical) included in the multicenter study, overall stroke rate was 4.2% (95%CI: 1.9-8.7). Systematic literature search identified two observational studies involving 576 patients that were StrokeJournal of the American Heart Association (JAHA)eligible for meta-analysis. Among 741 pooled COVID-19 patients overall stroke rate was 2.9%(95%CI: 1.9-4.5). Risk of acute stroke was increased for patients with severe compared to non-severe COVID-19 (RR 4.12, 95%CI 1.7-10.25;p=0.002) with no evidence of heterogeneity (I=0%,p=0.82). Conclusions: Synthesized analysis of data from our multicenter study and previously publishedcohorts demonstrate that severity of COVID-19 is associated with an increased risk of acute stroke, underscoring the necessity of neurological monitoring in patients infected with SARS-CoV-2.

ABSTRACT

BACKGROUND AND PURPOSE: Recent observations linked coronavirus disease 2019 (COVID-19) to thromboembolic complications possibly mediated by increased blood coagulability and inflammatory endothelial impairment. We aimed to define the risk of acute stroke in patients with severe and non-severe COVID-19. METHODS: We performed an observational, multicenter cohort study in four participating hospitals in Saxony, Germany to characterize consecutive patients with laboratory-confirmed COVID-19 who experienced acute stroke during hospitalization. Furthermore, we conducted a systematic review using PubMed/MEDLINE, Embase, Cochrane Library and bibliographies of identified papers following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines including data from observational studies of acute stroke in COVID-19 patients. Data were extracted by two independent reviewers and pooled with multicenter data to calculate risk ratios (RRs) and 95% confidence intervals (95% CIs) for acute stroke related to COVID-19 severity using a random-effects model. Between-study heterogeneity was assessed using Cochran's Q and I2 statistics. International Prospective Register of Systematic Reviews registration number: CRD42020187194.
